Here is what you will need when you make an online appointment:

It will ask you to register with us - please provide us with

1. Your First Name
2. Your Last Name
3. A valid 10 digit Phone, area code first - We will call you between 6-8 pm 2 days before your appointment to confirm or reschedule.
4. A chart number (You can make it any number you wish)- for recall purposes in case you need to reschedule your appointment.
5. (Optional) An email address so we can send you an appointment confirmation.

The next couple of screens will ask you more information about yourself including if you have insurance and what is the reason for your visit.

Then you will see a schedule of available appointments. Just click on the time you would like and confirm your appointment. It's that easy!

If the time you would like to come is booked, call us at 404-352-3414, and we will try to fit you in close to your desired time.

Please bring your insurance card, your glasses, and contact lens prescription or boxes with you.
